- Complete a CFS Release of Information to new provider OR
- Complete your new provider’s release form and have is sent to our office.

Release of Information
Once a complete and valid Release of Information (ROI) is received, records will be sent within 15 business days. If you, or your requestor, do not provide a valid and complete ROI we will let you know.
Records will be kept until December 13, 2029.
