Records Requests Prior to December 13th
Prior to our closure on December 13th, you may obtain your Center for Solace records request through the following channels:
- Complete a CFS Release of Information to new provider OR
- Complete your new provider’s release form and have is sent to our office.

Send Post-Closure Records Requests (After December 13th)
Clients may download their records from the client portal until February 1, 2025.
Please have the referring agency or provider send a completed release of information and records request, including your signature to:
